Financial Assistance Program
Family Care Center is committed to helping people who are underinsured or unable to pay for medically necessary care. We never want your household’s financial situation to prevent you from seeking or receiving the care you need.
Who Qualifies
Financial Assistance eligibility is determined using household size, gross household income, Federal Poverty Guidelines (FPL), and insurance status.
How to Apply
Download the Financial Assistance Application (PDF), or get a copy by from your local clinic. Print and complete the form, then bring it to your scheduled appointment. Or fill it out digitally and email it to PatientPayments@fccsprings.com.
